2. Minnesota Language Survey
Minnesota is home to speakers of more than 100 different languages. The ability to speak and understand multiple languages is valued. The information you provide will be used by the school district to see if your student is multilingual. In Minnesota, students who are multilingual may qualify for a Multilingual Seal upon further assessment. Additionally, the information you provide will determine if your student should take an English proficiency test. Based upon the results of the test, your student may be entitled to English language development instruction. Access to instruction is required by federal and state law. As a parent or guardian, you have the right to decline English Learner instruction at any time. Every enrolling student must be provided with the Minnesota Language Survey during enrollment. Information requested on this form is important to us to be able to serve your student. Your assistance in completing the Minnesota Language Survey is greatly appreciated.
Language use alone does not identify your student as an English learner. If a language other than English is indicated, your student will be screened for English language proficiency.
All data on this form is private. It will only be shared with district staff who need the information to best serve your student and for legally required reporting about home language and service eligibility to the Minnesota Department of Education. At the district and at the Minnesota Department of Education, this information will not be shared with other individuals or entities, except if they are authorized by state or federal law to access the information. Compliance with this request for information is voluntary.
3. Permission Trip, Waiver Form
As a parent/guardian, I give permission for my child to participate in all off-classes and field trips associated with Notre École, including transportation to and from off-site classes and field trips. These programs are not required by Notre École. I am aware of the inherent risks associated with these activities and that all risks cannot be prevented. If my child should require emergency medical treatment, I consent to such treatment. I acknowledge that the school district does not provide accident or health insurance for students, and I agree to be responsible for any medical bills incurred as a result of emergency medical treatment. To the maximum extent permitted by law, I hereby hold harmless and release Notre École, its officers, employees, and volunteers, from any claims for damages or injury to my child or property, which may arrive from my child’s participation in any program and all related activities.This release and holds harmless clause does not apply to intentional or negligent acts of Notre École, its officers, employees, and volunteers that cause harm to my child. However, by signing below, I agree that school district will not be responsible to pay any deductible on any insurance our family may have, if a claim is made that arises out of my child’s participation in any program.
4. STUDENT INTERNET USE / ACCESS PERMISSION FORM
I understand that internet access is designed solely fo educational purposes, and that it is intended that these resources are used only for educational purposes. Notre École has taken reasonable precautions to supervise internet usage by students. I have read and understand the Guide lines for Telecommunications Use which can be find here.
PARENTAL CONSENT
As a parent or guardian, I recognize that it is impossible for Notre École to control access by the students to all information or materials available on the internet; it is likewise impossible to limit disclosure of information related to school internet websites or publications by the larger internet public. I will not hold the school responsible for materials acquired, contact made, or for any limit on the educational privacy of my child as a result of the disclosure of information on the internet. I accept full responsibility for supervision of my child outside the school setting.

I hereby consent for my child to utilize the school internet services, and for disclosure of the following information when related to an activity or an academic assignment within Notre École, in accordance with the Family Educational Rights and Privacy Act, 20 USC §1232g:

5. Student Directory Permission Form
Dear Parents,
A new student directory will be created this year for the benefit and use of our school families.
Included will be your student’s name, address, phone number, parents name(s), and home email.
Neither this nor any other personal information related to students or parents is shared with individuals or parties outside of Notre École.
